What you will need: • U.S. Passport – You must provide your signed passport. Your passport must be valid for at least 6 months and have a blank page available. • Belarus Visa Application Form – Please provide a completed visa application form; click here for a .pdf of the application. Because the application changes on occasion, we’ve also provided a link to the Belarus Embassy website so you may get the most recent version directly from the Embassy website. Applicants will have to print a copy of their application form, sign and date it. **Please note – If a question does not apply, please type “N/A”. If the answer is none, write “NONE”. Incomplete forms will be returned to the applicants UNPROCESSED.. • Visas & Authentication Visa Request Form – filled out completely. • Photograph – Please provide two color, passport size photo. The photograph must be printed on photo paper, it must be 2″ by 2″ and have a white background, and it must have been taken in the last 6 months. • A copy of a letter of invitation from a belarusian travel agency. Hosting organization should be registered as a tourist company by the Belarusian authorities. • Copy of your airline ticket or travel agent itinerary. • Medical insurance – All visa applicants have to present travel insurance valid for Belarus or worldwide. A letter from applicant’s insurance company (original) is required, accompanied by the applicants insurance card. • **Please note that a photocopy of insurance card will not be sufficient enough to meet the above requirements.

Amount of emergency medical coverage (should not be less than $12, 000 with $0 deductible). **Please note that a photocopy of insurance card will not be sufficient enough to meet the above requirements.

Belarus Consular Fees: • Single Entry Visa (3 months) – $70 • Double Entry Visa ( 3 months)- $105 • Multiple Entry Visa ( 3 months)- $140 • Multiple Entry Visa (up to 12 months)- $170 **Expedited processing for these services incur an additional 100% charge by the Embassy.
